Below are copies of the 2023 – 2024 NCHSAA physical and PPE form. These forms will need to uploaded into the athletes dragonfly account.

ALL paperwork should be filled out on Dragon Fly or a picture taken and uploaded to Dragon Fly.
How to connect your DF account to another school
Physicals and Medical Eligibility forms should be uploaded to Dragon Fly by taking a picture of them and uploading.
Packets expire 395 days from the date of the last physical
Only current year (2023-2024) physical packets will be accepted.
