SPORTS TRYOUTS REQUIREMENTS

ALL students planning to try out for sports must complete and submit the Middle School Athletic Packet.

Students must have an up-to-date completed physical form submitted (within the last 12 months) before they are allowed to tryout. Please sign up in DragonFly to set up an account and upload ALL of your completed forms.   

A completed paper copy of the forms will be accepted. Please turn ALL forms in directly to Coach Wormack ONLY!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

INCOMPLETE FORMS WILL NOT BE ACCEPTED!!!!

A student must have insurance coverage to participate in interscholastic sports.  Students wishing to apply for school insurance may pick up the application form from  or in the Coach Wormack in the gym.

Students must meet attendance, academic, and discipline standards to be eligible for interscholastic athletic participation.
